Fisher-Titus Medical Center's Prescription Assistance is a community benefit program of Fisher-Titus Medical Center. We have a number of programs that may assist you with your medications.
Do I Qualify?
- No prescription insurance coverage?
- Difficulty affording your prescriptions on a monthly basis?
If you answered yes to either of these questions, you may qualify for assistance.
What Should I Bring?
- When you come for an appointment, you need to bring: Proof of income - federal tax return and proof of your household monthly income (salary, pension, Social Security for all members of the household).
- Medication bottles or prescriptions - of all medicines.
We cannot help you without the proper information. All information will remain confidential.
After the form has been completed it will be given to you to take to your physician's office for signature and mailing.
How Will I Get My Medication?
If you qualify for assistance, you will be given instructions on how to get your medications.
