Fisher-Titus Medical Center offers several different levels of assisted care.

The Home Health Center is a comprehensive health program designed to provide you with skilled health care in your own home. Your treatment will be planned by your physician and will be supervised by a Home Health Center nurse and/or therapist.

The award-winning Home Health Center provides services to residents of Huron, Erie, Sandusky and Seneca counties, as well as portions of Ottawa, Lorain, Richland and Ashland counties. Home Health Center nurses provide care for patients of all ages, including the pediatric patient who needs weight and wellness checks or the adult recuperating from injury or illness or has a chronic condition.

The Carriage House and Norwalk Memorial Home's range of services to older adults are integrated with a commitment to personal wellness and meeting each resident's individual needs. The care provided is driven by the goals that each resident holds important. By continuing to listen and respond to each resident's needs, we have proudly earned a reputation for quality in all we do.
