The Home Health Center provides services to residents of Huron and Erie counties, as well as portions of Sandusky, Seneca, Lorain, Richland, Ashland, Crawford and Ottawa counties. Home Health Center nurses provide care for patients of all ages, including the tiny pediatric patient who needs weight and wellness checks or the adult recuperating from injury or illness, or has a chronic condition.
In partnership with you and your physician, our health care team will design an individualized care plan to meet your health needs. This plan will include one or more of the following services: Professional nurses providing a variety of services including the targeting of health care needs, skilled nursing care and general health care information. Physical, occupational, and speech therapists will evaluate needs and develop a home rehabilitation program in cooperation with your family physician. Licensed practical nurses and home health aides will meet needs such as bathing, and hair and skin care.
The Home Health Center staff provides physical therapy services to patients recovering from orthopaedic surgery, stroke, or other conditions which leave the patient physically challenged. "High Tech" services, such as in-home IV therapy, are important components of care provided by the Home Health Center. A registered dietitian is available over the phone to plan special diets and provide nutritional counseling as ordered by your family physician. Postpartum care is available to assist new mothers in caring for themselves and their newborn. A Social Worker is available to evaluate community needs or to assist with long term care placement.
Payment for service is covered by Medicare, Medicaid, private insurance, Workers Compensation, and many private insurance providers as well as self-payment. Our staff will be happy to work with you regarding financial arrangements.
The Home Health Center is a fully certified provider of home health services. The agency is certified by Medicare and the Ohio Department of Health. In addition, the Home Health Center contracts with many major health insurance companies, as well as accepting private payment for its services.
