Blood Thinners & Blood Clot Management
The Anticoagulation & Thrombosis Management Clinic is where patients can get the help they need to monitor and manage the medication they are taking to prevent or manage blood clots. Patients must have a physician referral to access the services of the Center. Prescription assistance is available.
Our Clinic can provide patients taking Coumadin or oral thrombin inhibitors with:
- Scheduled blood tests for Coumadin (warfarin) dosing, monitoring and management
- Prescription refills for Coumadin/warfarin
- Education about possible drug and food interactions
- Regular blood pressure checks
- Direct communication with Primary Care and Medical Community
- Point-of-care testing
- Education and consultation with patients, caregivers and providers
For DVT patients, the Clinic provides:
- Guideline-based initiation, monitoring and discontinuation of appropriate anticoagulation therapy
- Coordination of anticoagulation therapy including ordering lab tests, adjusting anticoagulation medications, monitoring adverse reactions and compliance
- Initiation of compression therapy to prevent Post-Thrombophlebitic Syndrome
- Initiation of hypercoagulable work-up if appropriate
- Education and consultation with patients, caregivers and providers
