Blood Thinners & Blood Clot Management

The Anticoagulation & Thrombosis Management Clinic is where patients can get the help they need to monitor and manage the medication they are taking to prevent or manage blood clots. Patients must have a physician referral to access the services of the Center. Prescription assistance is available.

Our Clinic can provide patients taking Coumadin or oral thrombin inhibitors with:

  • Scheduled blood tests for Coumadin (warfarin) dosing, monitoring and management
  • Prescription refills for Coumadin/warfarin
  • Education about possible drug and food interactions
  • Regular blood pressure checks
  • Direct communication with Primary Care and Medical Community
  • Point-of-care testing
  • Education and consultation with patients, caregivers and providers

For DVT patients, the Clinic provides:

  • Guideline-based initiation, monitoring and discontinuation of appropriate anticoagulation therapy
  • Coordination of anticoagulation therapy including ordering lab tests, adjusting anticoagulation medications, monitoring adverse reactions and compliance
  • Initiation of compression therapy to prevent Post-Thrombophlebitic Syndrome
  • Initiation of hypercoagulable work-up if appropriate
  • Education and consultation with patients, caregivers and providers
