Diabetes Prevention & Management

If you have diabetes, management services at Fisher-Titus Medical Center can improve your quality of life. We offer diabetes management services that include education classes, support, and resources to help combat a disease that affects millions of Americans. Complications from diabetes can lead to heart disease, kidney disease and blindness. If left untreated, diabetes can even be fatal.

If you have a parent or sibling with diabetes, or if you experience one or more of the following symptoms on a regular basis, talk to your doctor:

  • Excessive thirst
  • Frequent urination
  • Extreme fatigue
  • Unexplained weight loss
  • Occasional blurred vision

Diabetes Self-Management Education

The Fisher-Titus Diabetes Self-Management Education (DSME) program is designed for individuals interested in learning more about diabetes management. You will receive a one-on-one visit with a registered nurse and dietitian and are encouraged to join group classes. If you are interested, talk to your doctor and they can refer you to the program.

The Self-Management Education program will help you:

  • Understand the diabetes disease process
  • Learn to identify the signs, symptoms, and treatment of high and low blood sugars
  • Discuss proper foot and eye care
  • Set personal goals and learn how to problem-solve
  • Review diabetes medications and how to take them safely
  • Learn ways to manage stress
  • Demonstrate proper insulin administration
  • Learn about available local and community resources
  • Learn to properly use home glucose monitoring equipment (free meters available)
  • Understand how to prevent, detect, and treat complications of diabetes
  • Learn how exercise helps manage diabetes
  • Discuss the benefits of being physically active
  • Assist in the development of customized meal planning
  • Be able to determine healthy food choices when grocery shopping
  • Set personal goals to improve eating habits
  • Learn to properly self-manage diabetes
